  A letter from the contractor and the consultant for the duration of the leave for a maximum of forty-five days. Copy of the license, work statement, workers, with a new date, for the engineer to pass the original licensed engineer card. Copy of the engineer’s card. A copy of the building permits in case of a request between a contractor and consultant.
